The safety of our kids is so important! Every teammate that serves with kids is required to go through a clearance process. This form gets that process started and gives us some other basic information we need for you to start serving.

Note: A criminal record will not necessarily prevent you from serving. Only offenses relating to minors would affect your ability to serve around kids.
