The safety of our kids is so important! Every teammate that serves with kids is required to go through a clearance process. This form gets that process started and gives us some other basic information we need for you to start serving. You must be at least 13 years old to serve on the Kids Team!

NOTE: A criminal record will not necessarily prevent you from serving. Only offenses relating to minors would affect your ability to serve with kids.
